Peer Support Consumer Application Form
The Peer Support Program offers social support groups and a range of community based activities. As these activities are supported by volunteers the person with a disability must have low to moderate support needs and be over 12 years of age.
Host Consumer Application Form
Flexible Respite Support – Host Family and In Home Support. This based on individual respite support needs and may include being linked with a volunteer/support worker or other alternatives.
